New Patient Registration

The practice boundary covers the majority of Keighley. Please contact the surgery and provide your postcode to check that you are eligible to register. If we are unable to accept you as a patient we will inform you of the reason.

To register with the practice please complete a Registration Form and bring into the surgery. You will also be asked to fill in the ethnic origin and smoking form and the alcohol screening questionnaire. You will also need to bring a form of identification with you when registering, preferably your passport.

New patients are registered with the practice rather than with a particular doctor. Patients have a right to express a preference for a particular practitioner, although you may have to wait longer to see your chosen clinician. Patients can be seen at either surgery depending on their preference.

Change of Personal Details

If you change any of your personal details please let the centre know as soon as possible.
